ESSENTIAL JOB FUNCTIONS:
- Provides interventions as needed to clinic’s patients within area of expertise and limits of credentials.
- Completes clinical assessments of patients to establish a foundation for treatment, and provide therapeutic interventions, counseling, and instruction about mental health and/or substance use needs for outpatient therapy.
- Maintain appropriate clinical documentation according to State and clinic standards.
- Works with patients on a day-to-day basis using professional and clinical judgment with discretion to assure that patients obtain needed services and appropriate care.
- Assists in the development, implementation, and revision of individual or family treatment plans; assures that services provided are specified in the treatment plan and monitors progress toward identified treatment goals.
- Identifies patient’s insurance coverage or other sources of payment for services.
- Identifies, monitors, and addresses patient’s risk factors and/or obstacles to care.
- Identifies patient’s needs, current services, and available resources, and then connects the patient to services and resources to meet the patient’s needs and goals of treatment.
- Communicates the care preferences of patients, serving as their advocate, and verifying that inventions meet the patient’s needs and goals of treatment.
- Serves as a liaison between the patient and their primary care provider to resolve care concerns as they arise and collaborates with the primary care provider to assure care is provided in an integrated manner.
- Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow-up with the focus on attaining treatment goals.
- Keeps abreast of new information and materials by reviewing clearinghouse information, newsletters, evidence-based practices, and periodicals.
- Researches and designs educational materials as indicated by best practices and patients’ needs to assist in developing and obtaining personal health goals.
- Perform all other duties as assigned.
EDUCATION:
- Master’s degree in social work, counseling, human services, or psychology from an accredited college/university is required.
- Certification or Licensure to treat Drug and Alcohol disorders preferred
- Training in crisis intervention and case management preferred.
REQUIRED SKILLS:
- Knowledge of signs and symptoms of mental health disorders.
- Demonstrated clinical decision-making skills appropriate to patient care and safety.
- Excellent written and verbal communication and critical thinking skills.
- Proficient with Microsoft Office Suite or related software.
- Ability to work independently and as a member of a care team.
- Ability to work well with individuals of diverse socioeconomic, cultural, and intellectual backgrounds.
PREFERRED SKILLS:
- Experience in use of Electronic Health Records.
- Understanding of integrated care models.
